Medical Coverage
The Gold Plus HMO (Plan 335-002) is an in-network only plan. You must use providers within Humana’s network. Find participating providers here: https://findcare.humana.com/
Emergency & Urgent Care
For life-threatening emergencies, any doctor or facility is considered in-network. Urgent Care Centers are also treated as in-network.
Prescription Coverage
New for 2026: The out-of-pocket maximum for covered prescriptions is $2,100. This applies to medications listed on the Humana formulary.
Dental Benefits
Your dental benefit works best with in-network dentists, but you may choose to see the dentist of your choice.

You have a $50 quarterly allowance to buy approved over-the-counter health and wellness products available through Humana's OTC Mail Order provider. The allowance is available to use the first day of each quarter - January, April, July, October.
